Repositioning

patran repositioning
  1. The illustrations are not exact in detail but promote understanding.
  2. Follow your hospital safety procedures for repositioning patients.
  3. Never pull directly on the PATRAN to move a patient. Use the sheet.
  4. The patient has slid down in bed and needs to be repositioned.
  5. Move the bed into the level flat position and LOCK THE WHEELS.
  6. Free the sheet from under the mattress or pad on all sides.
  7. With one person on each side of the bed, and using the sheet, pad, or chuck, log roll the patient on his side. The bed rail (not shown) should be up on the side the patient is rolled towards. If the patient cannot be log rolled go to step 8.
  8. Place the PATRAN next to the patient between the sheet and the mattress. Tuck the PATRAN under and past the patient to the other side, like changing sheets.
  9. If the patient can be log rolled and it’s easier, log roll them again as per the illustration and pull the PATRAN out on the other side. For extremely large patients, you may have to use two PATRANs.
  10. Straighten out the PATRAN so that it is under the patient’s entire body (or at least from just below the hips to the top of the head)
  11. Both caregivers stand towards the head of the bed.
  12. Roll up the edge of the sheet a little to provide a better handhold and prevent tearing.
  13. Pull on the sheet moving it and the patient towards the head of the bed (consult your safety or ergonomics advisors on the best posture for pulling).
  14. Move the patient into the desired position.
  15. Remove the PATRAN per step 12 on the other side of this sheet.
  16. Adjust the linen under the patient and tuck it under the mattress again.
  17. Reposition the bed as desired
  18. Fold the PATRAN and store it near the patient for reuse with that patient or throw it away (per established policy).
